Hazard and Incident Reporting

If you identify a hazard that can be fixed easily, take action immediately (e.g. pick up a trip hazard/clean up a spill). Other hazards may take more planning and consultation to resolve. If there is immediate or significant danger to people, move people to a safer area and notify the relevant supervisor.

If you experience a traumatic event or critical incident at work there is an Employee Assistance Programme (EAP) that provides professional confidential counselling. To get the best results this should be arranged soon as possible after the event - call 1300 360 364 or for more information see the HR webpage.

All hazards and incidents must be recorded on-line. Log in to myUNSW, click on the H2O symbol and follow the prompts. An email notification will go to whomever you identify as the responsible person. The responsible person will then conduct and record the investigation. Everything that is recorded online goes through the following process to track its status:

SUBMITTED Þ IN PROGRESS Þ ACTIONED Þ CLOSED

Hazard:
Hazard is a source of potential harm or a situation with the potential to cause harm to people, property or the environment.

Incident:
An incident is any unplanned event which occurs in the course of UNSW work which could result in:

  • Occupational illnesses;
  • Uncontrolled fire and explosion;
  • Physical injuries;
  • Equipment plant or property damage;
  • Dangerous occurrences which could have, but did not injure any person;
  • Exposure to hazardous substances or circumstances;
  • Minor injuries;
  • Any other incident that could put employees or plant at risk.
